How do you get it. I have reversed from mount (now that I know how to do it) but never get the trophy. Im pretty sure Im misunderstanding whats required. Any help guys?

Absolutely, reversal from situation means u sucked on the ground then reversed and won.
You must SWEEP (actively transition) to open guard from a mount defense position.
